The Hormonal Chaos Behind Cravings

When you’re trying to stay sober, and a craving hits, it can seem like a silent voice in your head, testing your willpower. And while the mental side can play a part, addiction cravings are also deeply physical. They are driven by hormones and brain chemicals that have been thrown out of balance by years of substance use. Understanding why cravings happen biologically can help you stop blaming yourself. You can then start seeing cravings for what they are, which is your body’s chemistry misfiring, not you being weak or failing in recovery.

Your stress system and how addiction hijacks it

Your body has a built-in stress response system called the HPA axis, which stands for hypothalamic-pituitary-adrenal axis. When you’re stressed, this system releases a chain of hormones that ends with cortisol flooding your bloodstream. Cortisol is sometimes called the “stress hormone”, and it is useful in short bursts because it helps you deal with threats. However, it is meant to switch off once the danger passes.

HPA axis addiction problems begin when you use drugs or alcohol over a long period. These substances artificially activate your stress system, forcing it to work overtime. Eventually, the system becomes locked in one of two states. For some people, it stays switched on all the time, pumping out cortisol even when there is no real threat. For others, it becomes so exhausted that it barely responds at all.

Stress, hormones, and relapse

Researchers looking at the link between cortisol, stress, and addiction have found that people in recovery often have cortisol that doesn’t behave the way it should. Some have too much of it, which keeps them in a constant state of low-level alarm. Others have cortisol that barely shows up when they need it, which leaves them feeling drained and numb.

Both patterns increase the stress hormone relapse risk. When your stress response is overactive, every difficult moment feels like a crisis, and your brain cries out for relief. When it’s underactive, you feel empty and disconnected, and substances start to look like the only thing that might bring you back to yourself.

In scientific terms, your neuroendocrine addiction system has been impacted. Your body has forgotten how to regulate itself, and until it heals, stress will keep pushing you towards relapse.

Dopamine and addiction cravings

Dopamine is the chemical that makes things feel rewarding and motivating. While cortisol is behind the desperate feeling in cravings, dopamine and addiction cravings explain the wanting. When you first started drinking or using drugs, substances flooded your brain with far more dopamine than anything in real life could produce.

Over time, your brain tried to protect itself by turning down its sensitivity to dopamine. It reduced the number of receptors available, which meant you needed more of the substance to get the same effect. You have probably heard this effect called “tolerance”. But it also meant that ordinary pleasures stopped feeling like anything. Food, friends, hobbies, and achievements that used to bring you satisfaction started to feel meaningless.

This is known scientifically as anhedonia, and it is why early recovery can feel so bleak. Your dopamine system is depleted, and nothing seems enjoyable. But your brain still knows that substances worked before, so it keeps pushing you to seek them out. This is the brain trying to find relief from the greyness that comes when your reward system has forgotten how to respond to normal life. By that point, it’s not even about feeling good anymore, but just about feeling anything at all.

The hunger hormones that make cravings worse

The link between drug cravings and neurobiology also has to do with the hormones that control appetite. Two of the most important are ghrelin and leptin. Ghrelin tells your brain you’re hungry and need to seek out food, while leptin tells it you’re full and can stop eating.

Research has shown that ghrelin and leptin are often affected in people with alcohol and stimulant problems. Ghrelin levels tend to be too high, which creates a constant sense of hunger and seeking. Studies have found that higher ghrelin is linked to stronger alcohol cravings and a greater risk of relapse. Leptin, which should balance out ghrelin, often fails to work properly.

What alcohol cravings biology tells us here is important. Your brain doesn’t just want alcohol for its effects on mood, but starts to want it in the same way it wants food when you’re starving. The craving can feel physical and deep in your body because it is driven by the same systems your body uses to keep you alive.

Blood sugar and addiction cravings

Another piece of the puzzle involves insulin and blood sugar. Chronic alcohol use, in particular, plays havoc with how your body handles glucose. The link between blood sugar and cravings in addiction is that drinking causes spikes and crashes, which your cravings follow like a roller coaster.

When blood sugar drops, you feel irritable, shaky, tired, and unable to concentrate. Your brain, which is desperate for fuel, sends out urgent signals to find a quick source of energy. For someone in recovery, these signals can easily be mistaken for substance cravings, or they can intensify cravings that are already there.

This is why so many people in early recovery and even those who quit smoking develop a sudden sweet tooth. Sugar provides the quick hit that their unstable blood sugar is demanding. But the cycle of spikes and crashes continues, and for some people, it makes staying sober much harder than it needs to be. Eating regular meals with protein and complex carbohydrates can help break this pattern and reduce the intensity of cravings.

Psychological versus biological cravings

There is a distinction between psychological and biological cravings, but in reality, they feed into each other. A psychological cue, such as walking past the pub where you used to drink, activates memories and emotions. But those emotions then activate your stress system, which releases cortisol, which makes your body feel like it’s in danger, which intensifies the craving into something that feels like a desperate need.

Part of hormonal imbalance addiction recovery is learning to recognise which kind of craving you’re dealing with. Psychological cravings often come with specific memories and images attached. Biological cravings tend to feel more like a general restlessness, an ache that you can’t quite locate, or a tension that won’t settle.

Both kinds of craving are real, and both are difficult to cope with and resist. But understanding that many of your cravings are being driven by hormones rather than weakness can help you ride them out without acting on them.

What this means for recovery

The good news is that your body can heal. The HPA axis can reset. Dopamine receptors can grow back. Ghrelin and leptin can find their balance again. Blood sugar can stabilise. None of this happens overnight, but it does happen with sustained sobriety and proper care.

In the meantime, knowing the biology behind your cravings can help you be kinder to yourself. When a craving hits, your chemistry is still recovering from the chaos that addiction created, and every day you stay sober, you’re giving yourself the chance to heal.
